A Clinical Evalution of the Adrenal Surgery

Abstract
This is a report of clinical analysis and evaluation of 13 cases of adrenalectomy experienced at surgical department of Catholic Medical College during past 8 years from 1969 to 1976.
There were 7 cases of Cushing¢¥s syndrome (5 cases of adenoma and 2 cases of bilateral hyperplasia), 1 case of pheochromocytoma, 2 cases of neuroblastoma, 1 case of metastatic mamm-ary carcinoma and 2 cases of invasion by surrounding malignancies.
4 cases underwent bilateral total adrenalectomy (3 cases of cushing¢¥s syndrome and 1 case of metastatic mammary carcinoma) through one or two stage and 9 cases underwent unilateral total adrenalectomy.
Surgical approaches performed were flank in 7 and transabdominal in 8 of 15 operations.
Four patients who received corticosteroid substitution therapy after bilateral total adrenalectomy have been in good health except one patient who developed hyperpigmentation of the skin.
Pneumothorax was only complication developed during surgery, and postoperative complications were other pulmonary problems, adrenal insufficiencies, wound infection and psychosis in order.
2 cases of neuroblastoma ended in sudden death possibly due to acute adrenal insufficiency and acute renal failure, accounting 15.4% of mortality in this series.
